Sedation Form

Requirements for boarding:
1. All animals must be current on all vaccinations (Rabies, DA2PP/L4, Bordetella).
2. All animals must be free of external parasites (ex. ticks, fleas, etc.), or they will be treated at owner's expense.

In case my pet requires emergency medical treatment, I understand the Caldwell Veterinary Clinic, PLLC will attempt to contact the appropriate person immediately. I further acknowledge that in the event of a medical emergency, the staff at Caldwell Veterinary Clinic, PLLC may not be able to contact the appropriate person immediately and therefore authorized to initiate treatment until the person can be reached.
