The adoption fee for your new pet is $50.00(Required) This price includes:
1. Veterinary Exam
2. Fecal Flotation to check for intestinal parasites
3. Strongid-T Deworming
4. Bath
5. Rabies Vaccination
6. Free 1 month dose of Heartworm prevention and flea control*
*Subject to a Heartworm test for adult dogs

Rabies Vaccination(Required) If your pet is older than 12 weeks old, we will administer the rabies vaccine. If not, then once the pet is old enough, you may bring the pet back to receive the rabies vaccine.

Other Vaccines(Required) We recommend fully vaccinating your new pet. These vaccines protect against contagious viruses. We also would recommend a Heartworm test for dogs older then 6 months and/or a leukemia test for all cats. The cost associated with these vaccines and tests is in ADDITION to the $30 adoption fee and at your discretion.
Yes, I understand vaccines and tests are in addition to the adoption fee.
Adoption Agreement Consent(Required) We want you to understand that this animal was a stray that was picked up by the City of Caldwell. Even though the veterinarian has examined this animal, it is possible from time to time for animals to carry and transmit diseases that can affect other animals and sometimes even people. These could include bacteria, viruses, parasites, and ringworm. These diseases may be undetectable in what appears to be a healthy animal at the time of adoption. We also want to stress to you that neither the City of Caldwell nor the Caldwell Veterinary Clinic, PLLC have knowledge of this animal's future health, disposition, vices, or its actions toward people and other animals.
I understand and accept these terms, and will not hold the City of Caldwell or the Caldwell Veterinary Clinic, PLLC liable for this animal, its health, its actions or transmission of disease from this animal to other pets or humans.
